Some of the comments on the EHI story mentioned in the post below are starting to discuss natural language processing - there's a commercial plug from a US organisation who have developed software which allows you to produce clinical coding for analytics (SNOMED, ICD-10) from unstructured text records.
Not sure of the applicability of that in the UK retrospectively or even currently, but future use of this kind of software to free clinicians could be interesting...
